Patients may be transferred from Secondary Care to Primary Care once established on a satisfactory dose regimen and that the patient is able to tolerate the dose.
The Specialist must provide the GP with a detailed clinical management plan including details of:

a) the possibility of future dose changes to control symptoms
b) a suitable dose range
c) expected review schedule
d) contact details should the GP require further guidance
